In order to enrol please come in to the medical centre with ID and a visa if required, you can print the enrolment form below and bring it with you or complete one at the practice. You need to be enrolled before we can book you an appointment.

Dr. Kirsten Crooke has been our family GP since sooner I moved to Wellington in 2016 and has an amazing combination of knowledgeability, thoroughness, and a kind and compassionate manner. I would highly recommend her to anyone looking for a GP for themselves or their children. I would also just recommend this practice more generally as they are responsive and caring. We have seen many other doctors there for last minute care appointments such as Dr. Garrett and Dr. Preston who are also both excellent.
